Important Information
Test Results – The doctors consulting from Ti-Tree aim to provide all patients with a copy of their test results on request by email once they have been reviewed.

We can email you on behalf of your doctor if you have completed a “Consent to use Electronic Communications” form available from reception or you can download from this website in Patient Forms.
Please register your correct email address in person, via the Hot Doc app or submitting the consent form.
X-Rays - Your x-rays are a valuable part of your medical record and should be kept by you safely at home. Please collect them from our reception staff. If you do not collect them within a month of being reported on – we will contact you on behalf of your doctor and advise you to collect them as soon as possible. They are unable to be permanently stored by your doctor, and if not collected within 3 months they will be destroyed.
Repeat Prescriptions - Please check your medication supplies before your consultation so that enough prescriptions can be issued until your next review. For most medications this can be a supply for 3 to 6 months. As it is not in the interests of best medical practice, we are unable to issue repeat scripts without consultation.
Parking - Parking is provided so there is easy access to the surgery. There is also additional space and direct access from the gravel carpark at the rear of the premises. We would appreciate it if you do not remain in the carpark if you are going to shop in the Village.
Medical Certificates - If you need a medical certificate, you must see your doctor when you are unwell. The law prevents your doctor from backdating a certificate or from issuing a certificate to a patient they have not seen.
Telephone Calls – Most problems are best dealt with during a consultation with your doctor. Sometimes it may be appropriate or necessary to speak with your doctor on the telephone. Our receptionists will pass on your message to the doctor, who will return your call as soon as practical. If you think the nature of the problem is urgent, please inform the receptionist who will put you through to the doctor immediately.
Documents - Official documentation including insurance applications, Vicroads Assessments, Disabled Parking Permit applications and Taxi Program applications require an appointment with your doctor to ensure these forms are completed accurately and in your best interests.
Referrals - Referral to a specialist requires a letter from your doctor explaining the problem. These are best discussed during a consultation so that your doctor can make the most appropriate decisions. The referral must be made prior to the specialist visit.
Vaccinations - We are able to provide a full vaccination service for infants, children and adults. After your vaccination, you will be asked to wait in the surgery for 15 minutes to guard against an allergic reaction. You may book to see your doctor for your or your child's vaccination, or if it is a routine vaccination you may make an appointment for the Registered Nurse to administer. Your doctor will charge their normal consultation fee, or a fee of $30.00 will be charged if you see the practice nurse ($25.00 if you are a pensioner). Details of your child's vaccination are routinely forwarded to the Australian Childhood Immunisation Register.
Reminder Systems – We support doctors who are committed to preventative care. You may be issued with a reminder notice from time to time offering you preventative health services appropriate to your care. You may be reminded by SMS, phone call or letter. If you do not wish to be part of this system please let receiption know.
Workplace Immunisations – We are able to offer a workplace immunisation program, where our Practice Nurse will come to your workplace and immunise your staff against Influenza and/or Hepatitis B.
Medical Students – From time to time we have medical students from Melbourne University or Monash University visit the Practice as part of their curriculum. Your doctor will ask you for permission to allow the student to observe your consultation. If you do not wish them to, please let your doctor know. It is your choice.
Melbourne Pathology – Blood collection and other pathology services are offered at Ti-Tree. This service is available Monday to Saturday.
Registrar – As part of Ti-Tree Family Doctors' passion and support for the development of community GPs, we are proud to support a GP Registrar. A GP Registrar is a fully trained doctor who has had several years of specialized experience working in a range of hospital settings. He or she is supervised and supported in their integration into the community setting of general practice. This provides an opportunity for the GP Registrar to be involved in the exchange of knowledge and ideas with well-established practitioners,that enhance the overall quality of care you receive.
